Shift: Day 5x8-Hour (08:00 - 16:30)

Description:
The RN Case Manager is responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience. Will be responsible for completing timely Initial Assessments, updated notes, and final notes. Will complete referrals to post acute levels of care. Will coordinate with the multidisciplinary team to ensure the discharge is on track.

Requirements:
CA RN Licensure
EPIC Experience
2 years of experience with knowledge of post-acute levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute and Acute rehab
Rotating weekends,
No on call required.

Mandatory Skills:
2 years of experience with knowledge of post-acute levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute and Acute rehab
