Case Manager
The main responsibility of this role is to provide case management services for a caseload of approximately 22 clients. The role requires the preparation of comprehensive assessments and service plans. Home visits and fieldwork are integral parts of the position.
- Conduct thorough assessments of clients' needs and develop personalized service plans.
- Maintain regular communication with clients to monitor progress and adjust plans as necessary.
- Coordinate with healthcare providers, social services, and other community resources to ensure comprehensive support for clients.
- Document all client interactions and maintain accurate records in compliance with organizational and legal standards.
- Participate in team meetings and contribute to the development of best practices in case management.
- Advocate for clients' needs and rights within various systems and institutions.
- Provide crisis intervention and support to clients as needed.
- Ensure confidentiality and privacy of client information at all times.
