Shift:05:45 PM to 06:15 AM

Minimumguarantee: 40 hrs

Summary:Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomesandpatient/familysatisfaction.He/sheoperateswithinthescopeofpracticedefinedby the State Nurse Practice Act.The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided byunlicensed staff.

Responsibilities:

  1. PatientEvaluation:

  1. Collects,reportsanddocumentsobjectiveandsubjectivedata;

  1. ObservesconditionsandreportschangesinconditiontoRN;

  1. CarePlanning:

  1. Contributestoestablishingindividualizedpatientgoals;

  1. Assistsindevelopinginterventionstoachievegoals;

  1. Implementstheplanofcare;

  1. Evaluateseffectivenessofinterventionstoachievepatientgoalsandminimizere-hospitalizations;

  1. Participatesinreviewandrevisionofplanofcare;

  1. ProvisionofDirectPatientCare:

  1. Administersmedicationsandperformstreatmentsperphysicianorders;

  1. CommunicatespertinentdatatoRNand/orphysician;

  1. Documentsaccuratelyandthoroughly;

  1. ConsultsandseeksguidancefromtheRNasnecessary;

  1. AtthedirectionoftheRN,participatesinUtilizationManagementorInterdisciplinary Care Management Meeting tofacilitateappropriate dischargeplanning:

  1. Communicatesteam'srecommendationsfordischargetotheattendingphysician,patientandfamily;

  1. Preparespatient/familyfordischargeincludingpatientandfamilyeducation;

  1. Monitorspatientcareprovidedbyunlicensedstaff:

  1. AssignsdutiesasspecifiedbyStateNursePracticeAct,clientpolicy,andstafftraining;

  1. Communicatespatientinformationwithunlicensedstaffduringandbetweenshifts;

  1. Monitors,evaluatesandfollows-uponassignedresponsibilitiesofunlicensedstaff;

  1. Ensuresthatassignedtasksareperformedinaccordancewithpoliciesandprocedures;

  1. UsesPointClickCare(PCC)accordingtotheBusinessProcesses;

  1. Performsdrugcountwhenbeginningandendingshiftandwhenevernecessary;

  1. Assistsphysicianinroundsandexaminationofpatients,asneeded;

  1. Participatesinshift-to-shiftcommunicationbetweenincomingandoutgoingnursingstaff;

  1. Collaboratesandcoordinateswithotherdepartmentstoprovidetimelyeffectivecare consistent with individual's needs,choicesandpreferences;

  1. Contributestoanenvironmentthatisrespectful,team-oriented,andresponsivetothe concerns of staff,patientsandfamilies;

  1. Enhancesnursingpracticebyattendingallmandatedin-serviceprograms;

  1. Assistswiththeorientationofnewlyhirednursingstaff;

  1. Promotesacultureofsafetytoensureahealthypracticeandlivingenvironment;

  1. MaintainsconfidentialityandprotectssensitiveProtectedHealthInformation(PHI)atalltimes;

  1. Staysandworksbeyondscheduledshiftifneededtomeetstatestaffingrequirementsand/or needs ofpatients;

  1. Performsotherdutiesasrequested.

Compliance:

  1. Complieswithapplicablelegalrequirements,standards,policiesandproceduresincludingbut not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.

  1. Participatesinrequiredorientationandtrainingprograms.

  1. Promptlyreportsconcernsandsuspectedincidencesofnon-compliancetosupervisor, Compliance Liaison or to the Compliance Officer via the Integrity Hotline.

  1. Cooperateswithmonitoringandauditfunctionsandinvestigations.

  1. Participates,asrequested,inqualityassuranceandprocessimprovementactivities.

SpecificEducational/VocationalRequirements:

  1. Graduateofanapprovedschoolforpracticalorvocationalnursingisrequired.

  1. MustbecurrentlylicensedbytheStateBoardofNursing.

  1. Experienceispreferredinrehabilitativeorgeriatricnursing.

  1. This position requires that the employeeisable toread, write,speakand understand the spokenEnglishlanguagetoensurethesafetyandwellbeingofourpatientsandvisitorsatthe work site when responding to their medical and physical needs.

  1. Must provide verification of TST (tuberculin skin test) as required bystate law andin accordancewithCompanypolicy.TSTswillbeadministeredattheworksiteifrequired.

  1. MaintainscurrentBLS/CPRcertification(AllSNFsandBehavioralCenters;ALFs/ILFsas required by State).
