Summary

This position provides case management for patient follow up for continuity of care for patients for all ages with chronic illnesses and for purchase referred care to non-IHS facilities on Contract funding. This position is located at Indian Health Service (IHS), Navajo Area Indian Health Service (NAIHS), Gallup Indian Medical Center (GIMC), Division of Nursing Services, Internal Medicine Clinic, Emergency Room, Pediatric Clinic, and Family Medicine Clinic, Gallup, NM.. Responsibilities

Works closely and communicates with providers, nurses, social services and other disciplinary team to update on patient care activities.
Develops and maintains disease specific registries for various patient with chronic illnesses.
Arranges specialty appointments and procedures for patients at other facilities; coordinates appropriate patient appointments for follow ups at Gallup Service Unit (GSU) upon discharges from Non-IHS Facilities. Must be able to work with the various alternate resources, includes Purchased Referred Care (PRC).
Organizes patient information, i.e., referral medical care documents, appropriate lab or x-ray information; documents of Public Health Nursing referrals or home health visits. Review medical information to determine the best plan of care with collaboration with multidisciplinary team.
Assists with ordering equipment and needed supplies for patients.
Performs comprehensive professional nursing duties. Performs chart reviews and other data collection and analysis activities clinically related to quality improvement activities.
Provides health education to patients with targeted illnesses in the form of brochures and direct teaching.

Measles and Rubella immunization required for selectees born after 1957. Seasonal Influenza immunization is required for civilian health care facility staff working in Indian Health Service health care facilities.

The selected individual is required to obtain and maintain an active, unrestricted Nursing license in any State (U.S.). If they are not obtained or maintained during employment, the employee will be subject to adverse actions, up to and including removal from Federal service.

Work maybe performed on a rotational basis on day shifts, including evenings, weekends, and holidays.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.

For positions requiring positive education requirements, or if you are using education to meet all or part of the qualification requirements, you MUST submit a copy of your transcripts or an itemized list of college courses which includes equivalent information from the transcript (course title, semester/quarter hours, and grade/degree earned) in your resume.

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-09 and above grade level, many positions may require experience in a specialty area of nursing.
Basic Requirements for the GS-11 (or equivalent) Grade Level
In addition to the above requirements, applicants must meet one of the following requirements:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience (equivalent to the GS-09 grade level) and possession of a diploma, associate degree, bachelor's degree, or maser's degree from a professional nursing educational program. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.

MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: experience performing a variety of activities involving, assuring continuity of health care from admission through discharge to home or another health care facility; identifying, mobilizing, utilizing, and coordinating resources to meet the patient's needs; ensuring that documentation by health care providers is complete and appropriate for patient's level of care/need. This involves much communication and know about available resources in and outside the community. Must be proficient with electronic health record, and various software for data extraction and use of various program for preparing records.

You must meet all qualification requirements within 30 days of the closing date of the announcement. Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
